J'NON GRIFFIN RN MHA HCS-D, COS-C, HCS-H

J’non Griffin is a 34 year veteran of home care as an RN. She received her Master’s Degree in Health Care Administration in 2005. She has experience as a field nurse, director and executive with home health and hospice agencies, both large and small. J’non is certified as a Homecare Coding Specialist-Diagnosis (HCS-D), Certified in OASIS competency, COS-C, Certified in Homecare Coding Specialist-Hospice (HCS-H), Home Care Specialist-Compliance (HCS-C) and is an AHIMA approved ICD-10-CM trainer/ambassador. J’non is an accredited ACHC and CHAP consultant. She has presented several webinars, and has been a nationally recognized speaker on a variety of subjects.

LYNN SMITH, RN, MSN

Lynn has over 28 years of clinical, management, and survey experience in the home health and hospice arenas. Lynn’s most recent position prior to joining SimiTree was as a full-time surveyor for home health, hospice, and palliative care for 9 years.

 

In her role as a Senior Manager, Lynn provides consultation and direction in maintaining agency licensure, accreditation, and coverage compliance requirements. Lynn conducts mock surveys and supports state, federal, and accreditation survey preparedness and plan of correction follow-up. Lynn assists organizations with staff training regarding Medicare eligibility and regulatory requirements and acts as a resource to clients and other SimiTree teams regarding state, federal, and accreditation compliance and regulatory updates.

 

Lynn is a CHAP Certified Consultant and an ACHC Certified Consultant. Lynn earned her B.S. in nursing from Indiana University of Pennsylvania and her M.S. degree in nursing administration from the University of Phoenix.

ROSALIND NELSON-GAMBLIN

Rosalind Nelson-Gamblin recently retired from a 33-year career in state government at the Texas Health and Human Services Commission, where she was a senior policy specialist for regulation of the home health, hospice, and personal assistance services industry including supportive palliative care, assisted living, and day activity and health services. In her state career, she analyzed state and federal legislation for impact to providers and consumers, promulgated and interpreted state regulations, developed and delivered continuing education and training to providers and surveyors, and provided regulatory guidance to both industry representatives and leaders and state of Texas surveyors and investigators.

Rosalind is also a subject matter expert for federal standards and possesses a keen understanding of accreditation for home health and hospice care. Rosalind is well-versed in the regulations for the settings where home care is delivered including nursing facilities, assisted living facilities, intermediate care facilities for individuals with developmental disabilities, and independent living settings.

Rosalind has been a public speaker for home care and hospice since 2000; and she is now an independent healthcare consultant.

TONY MELUZIO

Tony Meluzio is the Senior Content Manager for Axxess, the leading technology innovator for healthcare at home. Tony is responsible for the design and development of the Axxess Training and Certification Program. During his tenure, he has been able to advance Axxess’ educational products all while decreasing seat time for training while improving outcomes.

 

Tony comes to Axxess with over 20 years of experience in the healthcare space, with a primary focus on home health. He spent 10 years at the LHC Group beginning his career as a clinician and then advancing to corporate training where he designed and developed workforce management solutions, decreased onboarding time and turnover, created OASIS training, clinical specialty programs and survey readiness education. Tony also served as a consultant to other teams in the organization, detailing how to better deliver content and improve employee engagement. Since 2018, he has served as a professional consultant for EMR system optimization, survey readiness and documentation review.

 

Tony is a volunteer WVU Extension Master Gardener Program, where he leads community education efforts related to agricultural practices, sustainable living, raised bed gardening and a seed bank. He is also active in his local rotary chapter and the Pay It Forward Program, where he helps collect and distribute gently used items to people in need.

 

Tony holds degrees in physical therapy and healthcare administration from Fairmont State University and a master’s degree in instructional design and technology from West Virginia University.

LESLIE HEAGY, RN, COS-C

Leslie Heagy is Registered Nurse who joined Healthcare Provider Solutions, Inc. (HPS), as the Clinical Services Manager, in June 2017. Leslie is part of the clinical consulting team providing support and solutions to home health and hospice agencies across the country to achieve regulatory compliance.  In 2022, Leslie was promoted to Director of Clinical Services over both home health and hospice clinical consulting.

 

Leslie’s commitment to service and compassionate leadership first took root 21 years ago when she began providing care to patients as a hospice nurse after receiving her nursing degree from Ivy Tech Community College in Columbus, Indiana. Throughout her nursing career, she has worked in various capacities within the home health, hospice and private duty industries. Her experience includes successfully opening a Medicare/Medicaid licensed hospice start-up program, leading multidisciplinary teams in various locations to being part of the team to open a new hospice inpatient unit. She has educated hospitals, skilled facilities, assisted living facilities and the community on the benefits and requirements for home health and hospice services. Leslie’s extensive Federal and State Regulatory knowledge, along with her vast clinical experience motivates her to continually seek solutions to the barriers of healthcare in all settings.

 

Leslie has operational experience in the survey process for Medicare/Medicaid licensed agencies and has assisted agencies in developing/implementing corrective action plans. Her experience includes clinical consulting, performing clinical chart reviews, onsite billing audit reviews, provider education, developing and implementing quality and performance improvement programs for home health and hospice agencies.

SHEILA DAVIS

Sheila Davis is an accomplished Healthcare administration executive with comprehensive experience directing successful operational strategies and driving academic and research facility. Her background includes extensive years of excellent experience overseeing healthcare operations while spearheading financial reporting and management, budgeting, staff recruitment, revenue generation, and service line expansion. Ms. Davis was one of HomeCare University’s inaugural class, earning her certification as a Certified Home/Hospice Care Executive (CHCE). Additionally, she excels at propelling healthcare initiatives and policies while driving optimal facility efficiency and productivity within multimillion-dollar businesses. Ms. Davis serves on the NAHC Strategic Planning Committee, Advisory Council, Quality Data Implementation Committee, Rural Advisory Council, Finance Committee & Home Care Workforce Action Alliance; PDHCA Advisory Board; HCAOA Medical Advisory Board, Quality Data Review Committee & VA Committee; TAHC&H Board of Directors, Government Affairs Committee, Private Pay Committee, Membership Committee, and Community Care Services Committee. She is also a founding member of the Home Care Action Alliance; member of the Texas State Preventive Health Advisory Committee (SPHAC); serves on the WellSky Home Client Advisory Board; and is an ACHC Certified Consultant. Ms. Davis is engaged as the Senior Executive Vice President, Area Operations at Always Best Care Senior Services.

VICTORIA BARRON, MBA, BSN, RN, COS-C

Victoria Barron is a Registered Nurse who joined Healthcare Provider Solutions in December 2021 as a Clinical Consultant. As part of the HPS clinical consulting team, Victoria provides support and solutions to home health and hospice agencies nationally to achieve regulatory compliance. With more than 38 years of nursing and healthcare experience, the majority of Victoria’s career has been in the home services arena including home health, hospice, home care, and home infusion where she has served in various roles. In home health and hospice, Victoria has served as agency administrator, interim administrator, clinical director, CHAP surveyor, mock surveyor, QAPI team member and consultant. In private duty she has served as agency administrator, and in home infusion as marketing director and infusion Registered Nurse. Victoria is a licensed Multistate Registered Nurse and is a CHAP certified Home Health and Hospice Consultant.

Brian Lebanion has more than 25 years of experience in the healthcare industry and provides consulting services for all aspects of home health care and hospice, including operations, reimbursement and education. He specializes in comprehensive consulting services offering clients and providers survey/licensure coaching, clinical review and training, financial and revenue cycle management, third-party audit response, and quality assurance/performance improvement training.

JENNIFER AMHEISER, BSN, RN

Jennifer Amheiser has over 29 years of healthcare experience serving in acute care and community settings. As Director of Education Services at HealthCare ConsultLink, she coordinates the home health, hospice, and PAS educational events and also manages the online educational portal, EducationLink, set to be launched in a few short months. As a consultant with HCL, she assisted numerous agencies with QAPI processes, emergency preparedness plans, due diligence surveys, and start-ups. Jennifer especially enjoys educating agencies and helping with measures that directly impact patient care.

JENNIFER OSBURN, RN, HCS-D, COS-C

With nearly 30 years of Home Health experience, Jennifer uses her clinical field, software technology, and operations management experience to engage with home health professionals of all levels.

Her ability to bridge the gap between the field clinician and management’s understanding of regulation has led to numerous speaking engagement invitations including The Joint Commission and CHAP.

Jennifer is a Senior Manager of Operations Consulting at SimiTree, where she often creates and presents learning solutions for internal and external professionals. She holds dual specialist certifications for ICD-10-CM coding and OASIS.

JENNIFER PRESCOTT, RN, MSN, CDP

Jennifer Prescott is the founder of Blue Water Homecare and Hospice and a Registered Nurse. She is responsible for the strategic direction, vision and growth of the fastest growing independent Home care agency and Hospice in Central Texas.

 

As a seasoned professional with over 25 years of experience in nursing and clinical operations,  Jennifer is known for compassionate care for the elderly and maximizing operational excellence. She most recently served as Regional Executive Director at Altus Hospice where she increased sales 300% and tripled the average daily census in 2 1/2 years. Before that, Jennifer was Nurse Manager Operations at Duke University Hospital where she managed the daily operations of three ambulatory clinics serving 36,000 patients per year. During her tenure, initiatives Jennifer led resulted in a 75% reduction in fall rates during a six month period. Jennifer began her nursing career learning industry best practices at world class Children's Hospital of Philadelphia.

 

Jennifer holds a Masters of Science in Nursing from Duke University specializing in Healthcare Leadership and a Bachelor of Science in Nursing from University of Delaware. Jennifer proudly serves as an Executive Board Member for Capital Area Parkinson’s Society, and VP of Outreach and Education.  She is also on the Executive Board of The Alzheimer's Association Central Texas Chapter, Advisory Board member at Intuily, and Goodwill Industries Executive Healthcare Advisory Committee. Jennifer is a Certified Dementia Practitioner (CDP).  She is based in Austin, TX.

KAY HOLLERS, RN, MPH

Kay Hollers is well known to TAHCH audiences as she has been an active member or associate member since her return to Texas in 1985. Kay holds a Bachelor of Science degree from the University of Texas with a major in Nursing and a Master’s Degree in Public Health from the University of Texas School of Public Health. She also earned a post-master’s Specialist Certification in Health Care Evaluation from Wayne State University College of Nursing in Detroit. Kay taught Public Health Nursing at Michigan State University and has practiced in and consulted with home health and hospice agencies large and small, public and private. She currently is a consultant with HealthCare Consultlink. Kay likes to share her experiences and knowledge with audiences and has been a frequent presenter for TAHCH conferences and seminars.

COLLEEN BRYAN

Colleen Bryan has worked in the home care industry since 1996, holding various positions in both home health & hospice agencies including field nurse, case manager, educator, QA manager, DON and Administrator.  Colleen is currently a senior consultant and lead educator with JCC Consulting, and services home health and hospice agencies in Texas.

Colleen enjoys helping agency leaders navigate the constant changes and challenges in the current home health and hospice industries.  She enjoys assisting in the incorporation of meaningful and impactful changes into their agency’s operations – both administrative and clinical, in order to provide high quality care that is both sustainable while maintaining regulatory requirements and promoting excellent patient care.

CANDYCE SLUSHER, LVN, CDCS

Candyce Slusher, LVN is a geriatric-focused nurse serving seniors since 1995, working in every post-acute senior care setting, specifically in non-medical home care since 2004. 

As a Home Care Consultant her primary goal is to help agencies thrive while providing the highest quality care to seniors in their communities, while educating, empowering and supporting agency owners in all aspects of operations and compliance.

ANNETTE LEE, RN, MS, HCS-D, COS-C

Annette is a registered nurse, with a Master’s in Health Care Administration, practicing since 1990.  For over a decade she worked with the CMS Home Health and Hospice intermediary, Cahaba GBA and CGS, where she provided review of, and education on home health and hospice documentation.  She worked as a specialist in the Medical Review, Appeals, and Provider Education departments, and assisted with cross-MAC quality assurance and training.

  Annette then founded Provider Insights, Inc. providing consultative support and provider education regarding Medicare reimbursement issues and effective documentation strategies, assisting providers across the nation with Survey Preparation, ADRs and appeals. Today she continues to marry together her experience in the “real world” and the inside knowledge of Medicare to ensure providers can meet the requirements of CMS, and ensure compliant, efficient operations.

Valerie Cornett is the Chief of Strategy and Innovation for MAC Legacy and Home Health and Hospice Consulting Company.  Over the last 26 years, Valerie has developed a well-rounded understanding of the home health and hospice industry. Her diverse resume provides her with a unique insight and ability to communicate with clinicians, operators, and finance professionals.  Valerie is a strong behind-the-scenes problem solver.  Her passion is realizing a vision and seeing it through to completion.
